Current Status of Treatment Adherence to High-dose Methotrexate Regimens at the National Institute of Hematology and Blood Transfusion

Abstract
High-dose methotrexate (HDMTX) is an important component of many cancer treatment regimens but has the risk of causing serious toxicity, especially on the kidneys, liver and hematology. Therefore, strict adherence to HDMTX usage and monitoring instructions is necessary to ensure safety and treatment effectiveness for patients. The study was conducted with the aim of evaluating compliance with HDMTX treatment instructions at the National Institute of Hematology and Blood Transfusion. Subjects and Methods: A retrospective descriptive study was conducted on 214 patients who received high-dose methotrexate from January 2023 to June 2024, with a total of 382 treatment cycles. Results: The adherence rates to the HDMTX regimen regarding urinary alkalinization, fluid replacement, and leucovorin rescue were 91.10%, 29.84%, and 66.52%, respectively. Conclusion: Adherence to the treatment protocol remains suboptimal, indicating the need to strengthen compliance in clinical practice.
